Every effort has been made to ensure that all reasonable adjustments have been implemented to ensure that I can carry out my duties independently. Where this has not been possible my line manger and colleagues have been more than happy to assist.

There isn't enough space to explain the disability properly. Also a description of a disability on paper can sound very different, than a phone conversation or face to face meeting. Another thing is that potential employers still don't really understand where personal assistants come into play and that they can be the difference between being able to do a job or not.
